COMMUNITY STORIES

People deserve a place to be heard.

This is a moderated story project in progress. Submissions are reviewed before anything could appear publicly. A story here would never replace an official complaint.

PUBLIC STORIESNo stories have been published yet.

Your experience can help someone find the next step.

Share only what you can safely share. The first version of this form prepares a submission on your device; it does not publish or send it automatically.

PREPARE A TESTIMONIAL

Tell us what happened

Use plain facts. You may leave your name and contact information blank.

How publication would work

Before a future public version accepts submissions, it should use a secure server, spam protection, human moderation, a correction and takedown process, a clear privacy policy, and a way to report safety concerns. Stories would be edited only for privacy, clarity, and safety, with the author’s consent.

PREPARE BEFORE YOU CONTACT ANYONE

Make the record factual and usable

Write what you know, label what you heard from someone else, and preserve original documents safely.

  1. Write the basics. Date, approximate time, location, facility or agency, and names or identifying details you know.
  2. Separate observation from interpretation. Record exact words or actions when possible. Mark estimates and uncertainty.
  3. Describe disability-related needs. Communication method, mobility or sensory needs, medication or care concerns, and accommodations requested.
  4. Keep originals. Preserve messages, letters, photos, medical records, and witness contact information without editing the originals.
  5. Track follow-up. Record the office, date, person contacted, confirmation number, response, and next step.
ACCESSIBILITY IS PART OF REPORTING

You can ask for a usable process.

Ask for a communication method, interpreter, support person, extra time, accessible format, or other reasonable accommodation that helps you report safely. Be specific about what you need and keep a copy of the request.

If a process is inaccessible, record that barrier and ask the office who handles accommodation concerns.

SAFETY & RIGHTS

Report safely and keep control

Ask about confidentiality

Before sharing medical, disability, immigration, or other sensitive information, ask who can access it, whether disclosure is required, and how it will be used.

Document retaliation concerns

Write down threats, pressure, changed conditions, or adverse treatment after a report. Ask the receiving office how retaliation is addressed.

Use a trusted support person

You may choose to have a trusted person help organize facts or attend a call. Confirm the office’s rules before sharing another person’s private information.

What this site cannot promise: A report does not guarantee an investigation, a particular outcome, anonymity, or protection from every risk. For advice about legal deadlines or claims, consult a qualified attorney or legal aid organization.
